Advance Directives

Advance Directives is a legal document spelling out who your healthcare power of attorney (HCPA) is and what your wishes are regarding life-sustaining treatment. It is imperative that you complete them sooner rather than later.

Jacki will educate your in this process and assist you with completing the documents.

Even if you already have your Advance Directives, it is likely that they need updated, including a review of the decision-making process for HCPA selection.

Last Days Wishes

Putting your wishes in writing for how your last days are spent is a gift you give to your loved ones- and yourself.

Who do you want around you? Where do you want to be? These and other questions are answered as part of the process, as outlined in Jacki's book, Attending to the End: A Good Death Guidebook.

Schedule your appointment with Jacki today, to help guide you through these decisions and develop a plan for communicating your wishes to love ones.

Legacy Project

Have you thought about how you will be remembered by your friends and loved ones?

What do your friends and loved ones know about your life? Pivotal events. Cherished memories. The story of your life- across each decade.

There are many ways to share your story in permanent form- before you die.

Jacki can assist with assembling and creating the format of your choice.

Making death a part of life means we change our relationship with death by looking at it, with eyes wide open. By doing so, we also open opportunities for forgiveness, reconciliation and letting go of our attachments during life and at the end of life.

Death as a Part of Life: Living Mindful of Mortality

A Journey

All of us will die and while this is inescapable and thinking about it may feel overwhelming, denying it robs us of fully experiencing life. It also impacts our death.

The acceptance of our mortality influences not only our experience with death but also with life.

To live mindful of mortality doesn't mean we obsess over it.

When we live mindful of mortality, we live courageously, in a more conscious way. By nurturing our relationship with death we begin to prepare for it.

Jacki walks with you through the processes of deep reflection, life review, reconciliation and integration in the The Death as a Part of Life Journey.
